yeah they are different, time is a core concept. aging is just one of many effects caused by time and aging is just one of many reasons for death.
that is why time Is a level 5.. in this story the enemy follows a similar thing to how the espada was created in bleach, they represent a reason for human death.
in my own words time is a core concept of death and humans have always been a major reason of human(or just any life) deaths too thus they are both level 5.

disease, depression, gloom, are similarly causes of deaths for humans but more minor compared to something like time and human.
below that are level 3s like the hurricane thing we saw near the beginning of this story which represent natural disasters. Humans die from them yes but it is pretty rare.
level 2s are weapons which humans die by so it is even more minor (and weapons are just results from humans thus level 5 human)
