The author should have explained earlier how items can only be used in certain circumstances and why he couldn’t just pop recovery pills mid-battle. Kind of did, just didn’t make it very clear. They take time to work if he doesn’t use it with the system. If he uses it with the system, it activates the full effects immediately. But I don’t see why he couldn’t pop a pill (that was he carrying, not part of the system) and just recover bit by bit during the fight. Maybe it also messes up your energy circulation or w/e so that it’d not normally work in a fight. Only works because of the system, so he’s forced to wait for a system moment to use the recovery items.
